Mission

Program Objective:  Provide intensive group educational sessions that focus on nutrition, physical activity and emotional health to 30 children per year. Increase knowledge of nutrition, physical activity and emotional health among participants by a five percent increase in average knowledge score among participants at post-test compared to pre-test.

Action Plan: St. Louis Children’s Hospital currently provides the Head to Toe program twice annually to serve children from within St. Louis City as well as the surrounding community who have a written recommendation from their physician stating their need for the program. Expanding this program to St. Charles County will allow families to have access to the program closer to home. Pre- and post- assessment tools.

The National Diabetes Prevention Program—or National DPP—was created in 2010 to address the increasing burden of prediabetes and type 2 diabetes in the United States. This national effort created partnerships between public and private organizations to offer evidence-based, cost-effective interventions that help prevent type 2 diabetes in communities across the United States.

One key feature of the National DPP is the CDC-recognized lifestyle change program, a research-based program focusing on healthy eating and physical activity which showed that people with prediabetes who take part in a structured lifestyle change program can cut their risk of developing type 2 diabetes by 58% (71% for people over 60 years old).

Mission

The YMCA’s Diabetes Prevention Program helps adults at high risk of developing type 2 diabetes adopt and maintain healthy lifestyles by eating healthier, increasing physical activity and losing a modest amount of weight to reduce their chances of developing the disease.

In a classroom setting, a trained lifestyle coach will facilitate a small group of participants in learning about healthier eating, physical activity and other behavior changes over 12 months, beginning with 16 weekly one-hour sessions, three sessions delivered every other week and then eight monthly Sessions for added support to help them maintain their progress.  The two goals of the program are 7% weight loss and 150 minutes of physical activity a week.

Demographic
Participants who qualify for the program must be at least 18 years old, overweight (BMI > 25) and at high risk for developing type 2 diabetes indicated by a confirmatory blood value, a clinical diagnosis of Gestational Diabetes (GDM) during a previous pregnancy, or a qualifying risk score.
Goal
To prevent or delay type 2 diabetes. Hundreds of lifestyle change programs nationwide teach participants to make lasting lifestyle changes, like eating healthier, adding physical activity into their daily routine, and improving coping skills.
Impact
Participants will receive a notebook which contains worksheets and handouts for them to use in each of the sessions of the lifestyle intervention. Participants will also receive a weekly journal and be asked to track their food and physical activity during the program. Starting on Week 5 of the program participants will receive a free three-month membership to the Gateway Region YMCA.

Better Choices, Better Health: Diabetes This workshop includes one orientation and six weekly sessions. Living with diabetes can present many challenges, such as knowing how and when to eat, living your life so you can avoid complications and getting support you want from family and friends.

Living with Diabetes: Refresher Class This short refresher course is for those who have been diagnosed for several years but just need reminders, updates and resources to stay or get back on track.

Living with Diabetes: How to Enjoy the Holidays A dietitian will be on hand to give tips and advice for how to enjoy the holidays without breaking your treatment plan.

To get the latest information and dietary advice for their self management of their disease. Healthy behavior modification will reduce the risk of chronic complications.
Impact
Progress will be evaluated by measuring the number of survival skills assessed by patients who complete pre and post assessment tests. Further laboratory results both pre and post education can measure the clinical application of survival skill knowledge.

Chronic Disease Management: While diabetes is a serious disease and is costly, it is manageable. Living with diabetes requires constant tracking, careful meal planning and proper medication. Regular testing will help you keep track of your blood sugar levels.

New services through our Disease Management program will assist you and your medical provider in taking control of your well-being. Our certified diabetes educators will assist you in developing self-management plans and behaviors related to medications, diet, glucose monitoring, exercise and activity, sick day plans, problem solving for high and low blood sugars, carbohydrate counting and behavior modification.
